Our Services

Bloom Community Care (BCC) is a Registered NDIS Service Provider and complies with Government requirements for the delivery of quality and safe disability support services.

Bloom Community Care’s services cater for a range of eligible NDIS participants who can be either NDIS Managed, Plan-Managed or Self-Managed. Our aim is to provide the services that the participants need in order to be part of the social and economic life of their community, whether it be in making relationships and feeling connected to others, participating in community/social activities, increasing in confidence in mobility at home and around the community, or increasing in independence in carrying out self and home care activities.

Arranging and carrying out your NDIS Plan (Coordinator of Supports)

• Assistance in coordinating life stages, transitions and supports
• Coordination of all services needed to implement NDIS plans
• Ensuring Service Agreements and service bookings are completed on time
• Enabling participant choice and control over the supports and services they receive
• Assisting participants in preparing for NDIS plan reviews

Household Tasks

• Assistance with house and window cleaning
• Assistance with laundry & ironing
• Assistance with lawn and garden maintenance
•Assistance with meal preparation

Travel and Transport

• Transport to/from doctor and allied health appointments
• Transport to/from shopping
• Transport to/ from social and recreational activities

Other services available to facilitate capacity building and independence

• Assistance in daily personal activities
• Development of daily living and life skills
• Participation in community, social and civic activities
• Innovative community participation activities

With our multi-lingual staff and service focused support we ensure that participants are heard and have maximum control over the services they receive. Our open-door policy ensures that at any time, participants can give their feedback and know that they are heard.
